Do you work in Primary Care and would like to hear more about support for managing your hypertension targets?

Join the Academic Health Science Network for the North East North Cumbria (AHSN NENC) for a 45 minute information webinar on the UCLPartners Proactive Care Frameworks.


Staff working in GP practices and Primary Care Networks (PCNs) in the North East and North Cumbria are invited to attend the webinar to receive information on the frameworks, and find out more about resources that can support you.


About UCLPartners Proactive Care Frameworks

UCLPartners is one of 15 AHSNs supporting the health and care system to improve population health. On behalf of the AHSN Network, they are delivering a Blood Pressure Optimisation Programme which focuses on improving the management of hypertension, case finding for the undiagnosed and targeting health inequalities, in addition, to support for remote monitoring. It will do this by helping primary care to do things differently using the Proactive Care Frameworks developed by UCLPartners

AHSN NENC has been working with UCLP to spread these frameworks across our localities.

The frameworks will help practices and primary care networks to prioritise in this challenging time and focus resources on optimising care for patients at the highest risk. It will support the use of the wider workforce to deliver high quality proactive care and improved support for personalised care and release GP time.

The frameworks include:

  1. Comprehensive search tools to risk stratify patients – built for EMIS and SystmOne.
  2. Pathways that prioritise patients for follow up, support remote delivery of care, and identify elements of LTC care that can be delivered by staff such as Health Care Assistants and link workers.
  3. Scripts and protocols to guide Health Care Assistants and others in their consultations.
  4. Training for staff to deliver education, self-management support and brief interventions. Training includes health coaching and motivational interviewing.
  5. Digital tools and other resources that support remote management and self-management.
