Polypharmacy Project Call

The Academic Health Science Network (AHSN) and associated Patient Safety Collaborative (PSC) North East & North Cumbria (NENC) support a number of programmes around medicines, particularly approaches to addressing inappropriate polypharmacy and improving medicines safety.

Our programmes provide safety improvements across all healthcare settings.

Polypharmacy is a nationally mandated programme but we are also keen to ensure that we meet the needs of our regional member organisations and the STP/ICS.  We recognise that there are lots of opportunities to improve patient care around polypharmacy, and to make medicines safer for our patients in NENC.

With this in mind we are inviting expressions of interest from Clinicians and Teams within our region aiming to deliver innovative approaches to polypharmacy and associated medicines safety.

All projects will involve multiple organisations across the region. Any approach to addressing polypharmacy issues will be considered.

Successful projects will work with the AHSN Medicines Optimisation and Medicines Safety NENC team and be required to attend 6 monthly project leads meetings, to report metrics and to deliver their project against predefined milestones.  We are looking for projects to commence by 1 May 2019 and provide an interim report on outcomes by 1 November 2019.

Accountable to: Medical Director and the Medicines Optimisation Lead, AHSN NENC.

Please submit your proposal on no more than one side of A4 that defines how you would deliver the programme, approaches you would take, how you would use the funds and whether you would lead as an individual or as a team.

If you have any queries please contact Sue Hart AHSN Health Improvement Programme Manager email [email protected].
